We have over 20 years of experience in performing Immigration Exams for the USCIS.
Dr. Angela Adelman is a designated civil surgeon who is board certified in Internal Medicine. Dr. Adelman has been authorized by the Bureau of Citizenship and Immigration Services to conduct immigration exams for the Department of Homeland Security.
Services included in the $400 visit:
•Physical Exam, review of vaccinations, and completion of the I-693 Form •.Quantiferon Tuburculosis Test for all appliicants • Syphilis Test (RPR)- Required for applicants up to age 44, or possibly younger based on the physician’s determination. • Gonorrhea Urine Test for applicants up to age 24, or possibly younger based on the physician's determination.
(If the Tuberculosis test is positive, a chest X-ray will be required.)
• Upon completion of services on your follow up visit, you will be given completed and Sealed I-693 Form .
This envelope may not be opened and it to be submitted by you to the USCIS.
*An unsealed, identical set of documents will also be provided to you.
Vaccinations:
Bring your vaccination record with you if you have it. We can provide the Flu shot, MMR, Tdap, or Hep B vaccine here in our office if needed for an additional cost -or- we can check your blood titers for Measles, Mumps, Rubella, Varicella, and Hepatitis B if needed for an additional cost.
Required Vaccines:
MMR: at least one dose in lifetime if born after 1956
TdAP/Td: need 5 dose childhood series with a booster given in the past 10 years -or- a 3 dose adult series given in past 10 years or with booster given in past 10 years -or- 1 dose given in the past 30 days
HEPATITIS B: 2 dose of Heplisav B or 3 doses of Twinrix, Recombivax, PreHevrio, or Engerix-B at least 4 weeks apart until age 59 or 1 dose of any type given in the past 30 daysc
INFLUENZA: during flu season Sept 1- March 31
POLIO: need the 3 dose series or 1 dose given in the past 30 days
VARICELLA: reliable written or oral history of having the Chicken Pox disease or 2 dose vaccination series given as a child or 1 dose given in the past 30 days
PNEUMONIA: ages 65 and over
What To Bring:
• Valid, government issued picture ID such as a Valid Passport (preferred) or Driver’s License. (must match your USCIS paperwork - if maiden name listed bring marriage certificate or license)
• Medication list (if taking prescription medicines)
• Immunization records (translated to English or Spanish by an official translator, if necessary).
• Any mental health records (translated to English by an official translator, if necessary).
-Please bring a translator with you if you cannot speak English or Spanish
*Additional labs may be needed at an additional expense and may require up to 1 week for a result, depending on the test results.
*Immigration exams are NOT covered by insurance and are considered SELF PAY service. We will not submit any claims to insurance for payment.
